Term Life and Accidental Death & Dismemberment
(AD&D) Insurance

Blue Cross offers term life and AD&D insurance
for your employees along with medical and dental benefits. It is made
available through BC Life & Health Insurance Company. Moreover the
valuable coverage also proffers your employees by means of shielding
their family's financial future.
Convenience

Application. It is required that for both health
and life insurance one application is more than sufficient.

Administration. You have an extra convenience
of one consolidated monthly bill and less paperwork when you embrace
life insurance in addition to Blue Cross medical and dental plans.
This may be the easiest life insurance purchase you will make.

Guaranteed Issue.Coverage is a guaranteed issue
up to $50,000 to groups with 11 or more eligible employees. Also
groups with a number of 2-10 eligible employees or groups electing
to offer more than $50,000 are focused to medical underwriting.
Security
From any source it might be, the life insurance benefit
should be payable in the event of death at any time. The AD&D feature
pays an additional benefit if death results from an accident. The total
obtained is equal to the amount of the death benefit, subject to exclusions
and limitations.

These ratings symbolize the independent opinions of two of the insurance
industry's most respected rating agencies. You can even provide this
valuable benefit knowing that it is backed by the strength and stability
of one of the most respected names in the industry.
Blue Cross of California's affiliate, BC Life &
Health Insurance Company is offering the first year of the Premium Only
Plan (P.O.P) FREE to medical groups with ten or more lives that take
life benefits.
You may choose one of the following term life employee
benefit options:

*The job title descriptions shown are examples. You
may use them as a guideline, or prepare your own. There must be at least
one person in each class (job description). Only one benefit schedule
may be offered. Benefit amount for Class I cannot exceed $250,000 per
employee and cannot exceed 2.5 times benefit amount for Class II. **All
employees in Salary Based Life Insurance must have the same salary schedule.
In addition to the employee insurance, we offer the
following two dependent life options:
- $10,000 spouse, $10,000 children six months to 19 years (age 24
if full-time student), $1,000 for children under six months of age.
This option only available if employee life benefit is $20,000 or
more.
- $5,000 spouse, $5,000 children six months to 19 years (age 24 if
full-time student), $500 for children under six months of age.
The AD&D and Accelerated Death Benefit are not available
on either of the dependent life insurance options.
Additional Employee Term Life Benefits***
Accelerated Death Benefit
The Accelerated Death Benefit provides additional security
at a time when your employees may need it most. This benefit advances
up to 50 percent of the death benefit to an insured, while living, if
diagnosed with a terminal condition with 12 months or less to live.
Additional AD&D Benefits***
Accidents can come about at any time - so often previous
to your employees have had time to prepare for their family's future
- we provide the following benefits in addition to the loss-of-life
benefit:
- Education Benefit - $3,000 per year (to a
maximum of four years) to a dependent student in an accredited institute
of higher learning.
- Seat Belt Benefit - Suppose if the insured
dies in an auto accident, an extra 10 percent of the AD&D benefit
to transport to a maximum of $25,000.
- Repatriation Benefit -$5,000 maximum benefit
to transport mortal remains if the insured dies more than 75 miles
from home.
Eligibility
- Coverage is available to groups of 2-50 eligible employees.
- Eligible employees must convene the same full - and permanent
part-time employment requirements as health coverage.
- Sole proprietors, partners and corporate officers must work a
minimum of 20 hours a week.
Contributions and Participation

The employer must contribute at least 25 percent
of the total cost with a minimum of 75 percent employee's participation
who are eligible.

Guaranteed issue coverage entails 100 percent eligible
employee participation.

If the employer pays 100 percent of the coverage
cost, all eligible employees should enroll.

Only if the employee is paying some part of the
premium, an average of 75 percent of every eligible employee with
dependents must be enrolled and maintained in the dependent life
coverage.
